Abstract
การวิจัยนี้เป็นการวิจัยเชิงพรรณนา มีวัตถุประสงค์เพื่อศึกษาความชุกของภาวะซึมเศร้า ระดับ HbA[subscript 1]C และปัจจัยที่เกี่ยวข้องในผู้ป่วยเบาหวานชนิดที่ 2 ที่แผนกผู้ป่วยนอก โรงพยาบาลจุฬาลงกรณ์ จำนวน 250 ราย โดยวิธีการสุ่มตัวอย่างอย่างเป็นระบบ เครื่องมือที่ใช้ในการรวบรวมข้อมูลได้แก่ แบบสอบถามข้อมูล, แบบประเมิน Thai-HADS, แบบทดสอบ Thai Mental State Examination (TMSE), แบบประเมิน The Montreal Cognitive Assessment (MoCA) Test, แบบประเมิน The Summary of Diabetes Self-Care Activities (SDSCA), แบบสอบถามวัดเหตุการณ์ความเครียดในชีวิตภายใน 1 ปีที่ผ่านมา (Life Stress Event) และแบบประเมินบรรยากาศและความสัมพันธ์ในครอบครัว วิเคราะห์ข้อมูลโดยใช้โปรแกรม SPSS Window 11.5 สถิติที่ใช้ในการวิเคราะห์ข้อมูล คือ จำนวน ร้อยละ ค่าเฉลี่ยและส่วนเบี่ยงเบนมาตรฐาน, Pearson's Chi-square test, Independent t-test, One way ANOVA test, Pearson Correlation และ Multivariate Logistic regression ผลการวิจัยพบว่า กลุ่มตัวอย่างมีอายุเฉลี่ย 62.58 ปี เป็นเพศหญิงร้อยละ 64.4 และมีประวัติการศึกษาตั้งแต่ 12 ปีขึ้นไปร้อยละ 53.6 พบความชุกของภาวะซึมเศร้าในผู้ป่วยเบาหวานชนิดที่ 2 เท่ากับร้อยละ 28 และความชุกของการควบคุมระดับน้ำตาลในเลือดไม่ดี (ระดับ HbA[subscript 1]C ในเลือดตั้งแต่ 7% ขึ้นไป) ในผู้ป่วยเบาหวานชนิดที่ 2 เท่ากับร้อยละ 56 โดยในกลุ่มตัวอย่างที่มีค่า depressive scores ใน Thai-HADS มากกว่า 12 คะแนน พบว่าระดับน้ำตาลสะสมในเลือดมีความสัมพันธ์กับคะแนน depressive score แบบเส้นตรงในเชิงบวก (r = 0.803) นอกจากนี้ยังพบว่า ปัจจัยที่มีความสัมพันธ์กับการมีภาวะซึมเศร้า ได้แก่ภาวะวิตกกังวล ระดับการศึกษาน้อยกว่า 12 ปี ประวัติ admission จากโรคเบาหวาน ความเห็นว่าสุขภาพของตนเสื่อมลง ประวัติขาดยารักษาโรคเบาหวานและ diabetic nephropathy ส่วนปัจจัยที่มีความสัมพันธ์กับการควบคุมระดับน้ำตาลได้ไม่ดีได้แก่ การไม่มีโรคความดันโลหิตสูง การมี diabetic retinopathy ความเห็นว่าสุขภาพของตนเสื่อมลง การดื่มกาแฟ ค่า LDL ในเลือดสูงและการตรวจพบโปรตีนในปัสสาวะ
