Klinischer Verlauf und Krankheitslast durch RSV-Infektionen bei hospitalisierten Erwachsenen in Deutschland

Das Respiratorische Synzytial Virus (RSV) ist ein häufiger Erreger akuter Atemwegsinfektionen (ARI) und kann insbesondere bei älteren Erwachsenen zu schweren Krankheitsverläufen führen. Im stationären Sektor ist die individuelle und sozioökonomische Krankheitslast durch RSV-induzierte Erkrankungen noch unzureichend erforscht. BUCOSS Hosp – „BUrden of Disease and COst of IllneSS for RSV and other pathogens among Hospitalized adults” – ist eine prospektive, multizentrische Beobachtungsstudie, die über drei Infektionssaisons ARI-Patienten ( > 18 Jahre) aus 11 deutschen Krankenhäusern einschließt. Umfassende Daten zu Symptomatik, Komorbiditäten, Komplikationen, Therapie und Lebensqualität werden von Patienten mit RSV-Nachweis während des stationären Aufenthaltes sowie über einen Nachbeobachtungszeitraum von 90 Tagen erhoben. Patienten mit Influenza- und SARS CoV-2-Nachweis bilden die Vergleichskohorten. In der Saison 2023/24 wurden 807 Patienten (davon 84% > 50 Jahre) rekrutiert, von denen bei 32 (4%) eine RSV-, bei 96 (12%) eine Influenza- und bei 99 (12%) eine SARS-CoV-2-Infektion nachgewiesen wurde. Bei 30 (94%) der RSV-Patienten lag mindestens eine Komorbidität vor (Influenza: 88%; SARS-CoV-2: 91%), bei 21 (66%) eine chronische Lungenerkrankung (Influenza: 51%; SARS-CoV-2: 34%). Die Symptome Husten, purulentes Sputum, erhöhte Atemfrequenz und Rasselgeräusche wurden bei RSV-Infektionen proportional häufiger beobachtet als bei Influenza- oder SARS-CoV-2 Infektionen. Die durchschnittliche Krankenhausaufenthaltsdauer betrug bei RSV-Patienten 8 Tage (Influenza: 6 Tage; SARS-CoV-2: 7 Tage). Zudem kam es bei RSV-Patienten relativ häufiger zu einer Re-Hospitalisierung. EQ-5D-Quality-of-Life-Scores zeigten während des Krankenhausaufenthalts bei RSV-Patienten ähnliche, nach 28 bzw. 90 Tagen niedrigere mittlere Werte als in den Vergleichskohorten. Die Analyse der ersten Saison von BUCOSS Hosp zeigt, dass Patienten mit RSV-Infektion sehr häufig Komorbiditäten aufweisen, häufig schwerwiegende Symptome präsentieren, lange hospitalisiert sind und häufig re-hospitalisiert werden. Diese Erkenntnisse liefern einen wichtigen Beitrag zur Entwicklung von Strategien im Bereich der öffentlichen Gesundheit in Bezug auf ARI und insbesondere RSV-Infektionen.
