Ilgalaikė hormoninė kontracepcija ir depresijos rizika: sisteminė apžvalga ir metaanalizė

Abstract
Moterys, vartojančios hormoninę kontracepciją, neretai skundžiasi depresine simptomatika, tačiau ryšys tarp hormoninės kontracepcijos vartojimo, jos rūšių ir depresijos nėra aiškus.Tikslas – įvertinti literatūroje pateiktus duomenis ir nustatyti galimą ryšį tarp hormoninės kontracepcijos vartojimo ir depresijos rizikos.Tyrimo medžiaga ir metodai. Naudojant standartizuotą procedūrą buvo atlikta sisteminė literatūros apžvalga duomenų bazėse Pubmed ir Medline. Straipsnių tinkamumas buvo vertintas atsižvelgiant į jų kokybę, naudojant kokybės vertinimo skalę Newcastle-Ottawa. Apibendrintam poveikiui įvertinti taikyta atsitiktinių efektų metaanalizė, siekiant atsižvelgti į tyrimų heterogeniškumą.Rezultatai. Iš viso identifikuoti 193 straipsniai, iš kurių 9 atitiko įtraukimo į analizę kriterijus. Visų įtrauktų tyrimų kokybė buvo įvertinta kaip labai gera. Mišrių efektų metaanalizės rezultatai parodė, kad sudėtinių hormoninių kontraceptikų vartojimas yra nežymiai susijęs su padidėjusia depresijos rizika (RR 1,37; 95 proc. PI: 1,00–1,73). Hormoninės spiralės naudojimas nebuvo reikšmingai susijęs su depresijos rizika (RR 1,03; 95 proc. PI: 0,72–1,35), o progestino kontraceptikų vartojimas buvo susijęs su šiek tiek padidėjusia depresijos rizika (RR 1,19; 95 proc. PI: 1,00–1,38).Išvados. Progestino ir sudėtinių hormoninių kontraceptikų, bet ne intrauterinės spiralės, vartojimas yra susijęs su padidėjusia depresijos rizika moterims. Siekiant geriau įvertinti šį ryšį, būtina atlikti daugiau aukštos kokybės tyrimų, kurie atsižvelgtų į skirtingas kontraceptikų grupes, vartotojų amžių bei vartojimo trukmę.
