Abstract
El artículo describe un nuevo patrón de bloqueo interauricular avanzado (BIA) atípico con morfología trifásica observado en el electrocardiograma. Se presentan dos casos clínicos de pacientes adultos mayores en quienes se identificó una onda P trifásica en derivaciones inferiores, característica del tipo IV recientemente descrito. Este patrón refleja alteraciones en la conducción eléctrica auricular, relacionadas principalmente con el retraso en la activación entre ambas aurículas. Además, el BIA se asocia con mayor riesgo de arritmias auriculares, especialmente fibrilación auricular, así como con eventos como accidente cerebrovascular y mortalidad. Por ello, el reconocimiento de estos nuevos patrones electrocardiográficos es importante para el diagnóstico y pronóstico clínico.
