Multimodal Prehabilitation in Bone Sarcomas around Knee Joint: A Prospective Feasibility Study

Background: The endoprosthetic reconstruction post bone sarcomas around the knee joint has affects an individual’s function and participation in daily life. Prehabilitation has better outcomes when compared with standard postoperative rehabilitation in oncological conditions such as breast and colon cancers. No studies have reported about prehabilitation in bone sarcomas around the knee joint. Objectives: The aim of the study was to evaluate the feasibility of the multimodal prehabilitation, and the objectives were to measure the occupational performance, emotional health, fatigue, and nutritional status of patients. Study Design: Feasibility study. Methods: A feasibility study of prehabilitation was conducted on 24 patients with bone sarcomas around the knee joint awaiting endoprosthetic reconstruction while on neo-adjuvant chemotherapy. Assessments were carried out on 1 day before the chemotherapy fraction and on the 21 st day after the last chemotherapy dose. Prehabilitation consisted of an individualized occupational therapy program and nutritional interventions. Outcome measures included occupational performance, emotional health, fatigue, and nutritional status of patients as measured by validated scores, including Canadian Occupational Performance Measure, Depression Anxiety Stress Scale-21, Multidimensional Fatigue Inventory-20, and Subjective Global Assessment (SGA), respectively. The baseline and the last assessment were compared using SPSS software version 25. Results: Prehabilitation was completed by 22 out of 24 patients. For statistical analysis purpose, the level of confidence was set at 95% and the level of significance as P < 0.05. Significant improvement was found in the level of participation in patients’ occupational performance depicted with mean ± standard deviation (SD) and P values (P[Performance] pre [mean ± SD]: 1.37 ± 1.18, post [mean ± SD]: 4.55 ± 1.55; P < 0.001) (S[Satisfaction] pre: 1.32 ± 1.13, post: 4.56 ± 1.55; P < 0.001), fatigue (pre: 65.77 ± 9.36, post: 36.77 ± 12.06; P < 0.001), and emotional health (D[Depression] pre – 5.27 ± 3.01, post: 1.45 ± 1.44; P < 0.001) (A[Anxiety] pre: 4.82 ± 2.17, post: 1.09 ± 0.92; P < 0.001) (stress: pre: 6.95 ± 3.26, post: 2.23 ± 1.98; P < 0.001). Nutritional status was maintained in 20 patients, deteriorated in one, and improved in one as assessed by SGA. Conclusion: Multimodal prehabilitation in patients with bone sarcomas around the knee joint awaiting endoprosthetic reconstruction is feasible. Outcomes showed improvement in occupational performance, emotional health, and reduced fatigue. Maintenance of nutritional status was observed.
