Abstract
Ventricular arrhythmias are a common complication years after corrective surgery for tetralogy of Fallot (TOF).1 Monomorphic ventricular tachycardia (VT) is most often due to macroreentrant circuits that involve well-defined anatomic isthmuses within the right ventricle (RV), bordered by nonexcitable tissue, such as surgical scars or patches and tricuspid and pulmonary annuli.2 Focal VT has also been described and frequently originates within the same anatomic isthmuses that are implicated in macroreentrant VT, reflecting the arrhythmogenic characteristics of diseased tissue marked by fibrosis and heterogeneous conduction.
