Se presenta el caso de un adulto con diagnóstico de Mielitis Transversa Longitudinalmente Extensa luego de haber presentado una infección viral por Dengue, un arbovirus de la familia Flaviviridae que se transmite entre humanos a través de un agente vector, el mosquito Aedes aegyptil, caracterizado por fiebre, cefalea pulsátil moderada intensidad asociada a dolor retroocular, astenia, dolor osteoarticular, náuseas, vómitos y dolor abdominal, dando positivo en las pruebas inmunológicas de la enfermedad de Dengue(NS1), Alrededor del 10.8% de los pacientes, puede presentar diversas manifestaciones neurológicas del sistema nervioso central y periférico como: encefalopatía, encefalitis, meningitis, enfermedad cerebrovascular, miositis, parálisis periódica hipokalémica, polineuropatía y Síndrome de Guillain-Barré o Miller Fisher (3 - 5). Las complicaciones neurológicas más comunes del dengue, la encefalopatía y la encefalitis presentan una incidencia estimada del 0,5 y el 6,2 % (1), que cursó 10 días después con trastorno esfinteriano que requirió, uso de sonda foley se agrega una paraplejía flácida y alteración sensitiva (manifestado como nivel sensitivo). Motivo por el cual fue internado en el Hospital Nacional Guillermo Almenara Irigoyen, Lima-Perú; con el diagnostico de mielitis transversa, y después de las imágenes se consideró una mielitis transversa longuitudinalmente extensa, La afección medular inflamatoria (mielitis) es una complicación poco frecuente del Dengue y se puede presentar como mielitis transversa (MT) o mielitis transversa longitudinalmente extensa (MTLE). (6) La MT se caracteriza por alteración sensitivo-motora desde el nivel medular afectado hacia caudal y compromiso esfinteriano; por otra parte, MTLE implica afección de 3 o más niveles medulares continuos. (6, 7), dicho paciente durante la estancia hospitalaria, desde su ingreso a emergencia, recibió 05 pulsos de Metilprednisolona, luego de 72 horas, al no tener respuesta clínica, se inició 05 ciclos de Inmunoglobulina intravenosa, que fue bien aceptada por el paciente, además de terapia física. Con dicho manejo médico, el paciente tuvo una evolución favorable de la función motora y sensitiva, siendo referido al Hospital San Isidro Labrador para continuar terapia física y reeducación vesical.
