The role of platelet-rich fibrin (A-PRF) in soft tissue healing in the treatment of odontogenic radicular cysts

Abstract
Platelet-rich fibrin (A-PRF) is the second generation of platelet concentrates and is characterized by its ability toaccelerate wound healing and promote tissue regeneration; it is widely used in dentistry. A-PRF stimulates osteogenicdifferentiation of dental pulp cells and periodontal ligament cells and can be used to replace bone defects. To date,there are insufficient studies on the effectiveness of using A-PRF as a transplant material for periapical bone defects,including in the treatment of odontogenic radicular cysts. The purpose of the study was to increase the healing rate of soft tissues after surgical treatment of odontogenicradicular cysts using improved platelet-rich fibrin (A-PRF). Material and methods. Thirty patients of both sexes aged 20 to 60 years with a diagnosis of odontogenic radicularcyst were divided into 2 equal groups. All patients underwent cystectomy as the main treatment; however, osteoplasticsynthetic material was used in group 1 patients after surgery and advanced platelet-enriched fibrin (A-PRF) wasused in group 2 patients to replace the bone defect after cyst enucleation. All patients were followed for 14 daysafter surgery; diagnostic measures included clinical examination, pain assessment using the McGill questionnaireand visual analogue scale. Results. The conducted study proved accelerated healing of the wound with less pronounced postoperative signs(pain, swelling) in the group of patients where A-PRF was applied. Conclusion. The use of improved platelet-rich fibrin in the treatment of cystic lesions appears to be a new therapeuticapproach that promotes faster healing and repair of soft tissues, while minimizing standard postoperative signsand complications, however, further studies with a larger sample size and monitoring of bone regeneration over alonger period are needed. Улучшенный фибрин, обогащенный тромбоцитами (A-PRF) стимулирует осте-огенную дифференцировку клеток пульпы зуба и клеток периодонтальной связки, может быть использовандля замещения дефектов костной ткани. На сегодняшний день недостаточно исследований эффективностиприменения A-PRF в качестве трансплантационного материала при дефектах периапикальной кости, в т.ч.при лечении одонтогенных радикулярных кист. Цель исследования: повысить скорость заживления мягких тканей после хирургического лечения одон-тогенных радикулярных кист за счет применения A-PRF. Материал и методы. Тридцать пациентов обоего пола в возрасте от 20 до 60 лет с диагнозом «одонтогеннаярадикулярная киста» были разделены на 2 равные группы. Всем пациентам была проведена цистэктомия,причем у пациентов 1-й группы после операции применялся остеопластический синтетический материал, а упациентов 2-й группы для замещения костного дефекта после энуклеации кисты использовался А-PRF. Всепациенты находились под наблюдением в течение 14 дней после операции, диагностические мероприятиявключали клиническое обследование, оценку боли по опроснику Мак-Гилла и визуальной аналоговой шкале. Результаты. Проведенное исследование доказало ускоренное заживление операционной раны с менеевыраженными послеоперационными признаками (боль, отек) у группы пациентов с применением A-PRF.Заключение. Использование A-PRF при лечении кистозных поражений является новым терапевтическимподходом, способствующим более быстрому заживлению и восстановлению мягких тканей. Его применениеминимизирует стандартные послеоперационные признаки и осложнения. Однако необходимы дальнейшиеисследования с бόльшим размером выборки и наблюдением за регенерацией костной ткани в течениедлительного периода времени.
