Abstract

Extensive destruction of the aortomitral curtain, often resulting from abscess formation or aortic root pseudoaneurysm, represents one of the most severe complications of infective endocarditis. This warrants extensive debridement to eradicate infection and complex reconstruction to restore function.1,2 First described in the 1970s, the “Commando operation,” has become a critical option for patients with end-stage endocarditis and perivalvular extension. When executed well, this technically demanding operation offers the best chance of infection-free survival and recovery in otherwise-dire circumstances.
