Abstract
Abstract This chapter summarizes the current status of cognitive behavioral therapies for insomnia (CBT-I). After presenting a conceptual model of insomnia, the core therapeutic components of CBT-I are described along with other emerging therapies such as mindfulness, acceptance and commitment therapy, and intensive sleep retraining. The authors present a summary of the evidence that has led CBT-I to be recognized as the treatment of choice in current clinical practice guidelines. Potential mechanisms of changes are discussed, as well as the emerging literature on digital therapies. The authors conclude with some practical and clinical considerations that should guide therapists in the implementation of CBT-I. Given that CBT-I is still underutilized and that not all patients actually respond to CBT-I, recommendations for future research are made with reference to the design of more personalized therapies and greater deployment of digital therapies to improve access to insomnia treatment.
