Antecedentes: La diabetes mellitus tipo 2 (DM2) es una enfermedad metabólica crónica que predispone a diversas complicaciones sistémicas, incluidas enfermedades orales como periodontitis, caries y xerostomía. La evidencia sugiere una relación bidireccional entre DM2 y patologías orales, pero persisten lagunas en la literatura sobre su prevalencia y factores de riesgo específicos. Justificación: La identificación de patrones clínicos y factores predisponentes de enfermedades orales en pacientes con DM2 es crucial para mejorar estrategias de prevención y tratamiento. Una síntesis de la evidencia disponible facilitará el desarrollo de intervenciones basadas en la mejor evidencia científica. Objetivo: Evaluar la prevalencia, manifestaciones y factores de riesgo de enfermedades orales en pacientes con DM2. Métodos (PRISMA): Se realizó una revisión sistemática siguiendo las directrices PRISMA. Se consultaron bases de datos como PubMed, Scopus y Web of Science, abarcando publicaciones entre 2010 y 2025. Se incluyeron estudios observacionales y ensayos clínicos que analizaron enfermedades orales en pacientes con DM2. Se excluyeron estudios en animales, revisiones narrativas y publicaciones sin acceso a datos completos. La calidad metodológica se evaluó mediante las herramientas ROB-2 y Newcastle-Ottawa. Resultados: De 1,200 registros identificados, 25 estudios cumplieron los criterios de inclusión. La periodontitis presentó una prevalencia del 60% en pacientes con DM2 mal controlada, y la xerostomía se reportó en el 45% de los casos. Factores como hiperglucemia sostenida y pobre higiene oral se asociaron significativamente con un mayor riesgo de enfermedad periodontal. Conclusiones: La DM2 se asoció con tasas elevadas de enfermedades periodontales y xerostomía, lo que resalta la necesidad de estrategias preventivas y terapéuticas específicas. Futuros estudios deben explorar intervenciones para mitigar estos efectos.
