Abstract
North America is one of the most diverse regions in the world, including how family medicine has evolved and is structured. It is characterised by decentralisation and devolution, fostering local autonomy to account for diversity. While family medicine is well established, especially in the United States and Canada, scaling up necessary reforms is difficult due to this devolution of agency. In Caribbean countries, family medicine is still nascent, and efforts at international collaboration are required despite political forces prioritising local needs. A firm understanding of foundational values is required in this time of flux to face these and future challenges. The promise of artificial intelligence risks diluting the contribution of family medicine. However, artificial intelligence also makes us reflect on which attributes of family medicine are routine and which ones are essentially human.
