I read with great interest the recent article by Waheed and Holmes titled “Do Vaccines Cause Postural Orthostatic Tachycardia Syndrome (POTS)? Review of Cases in the National Vaccine Injury Compensation Program” [1]. While the authors provide a valuable analysis of POTS cases within the National Vaccine Injury Compensation Program (VICP), I believe critical scientific aspects related to causal inference, POTS, and vaccine safety were underemphasized or overlooked. Most notably, the article concludes with a recommendation to reconsider whether cases of purported vaccine-induced POTS should continue to be adjudicated in the VICP—a suggestion that, in my view, is both premature and insufficiently supported by the evidence, and one that risks undermining essential protections for individuals affected by rare but serious adverse events.
