Uso de agonistas del receptor GLP-1 en adultos sin diabetes para reducir eventos cardiovasculares mayores: revisión sistemática de la evidencia clínica

Antecedentes: Los agonistas del receptor de péptido similar al glucagón tipo 1 (GLP-1RA) han demostrado reducir eventos cardiovasculares en pacientes con diabetes tipo 2 [1–3]. No obstante, hasta fechas recientes no existía evidencia directa en población no diabética con enfermedad cardiovascular establecida. Objetivo: Evaluar sistemáticamente la evidencia clínica sobre el uso de GLP-1RA en adultos sin diabetes para la prevención de eventos cardiovasculares mayores (MACE). Métodos: Se realizó una búsqueda sistemática en PubMed, Embase, Cochrane CENTRAL, Web of Science y Scopus desde enero de 2018 hasta el 1 de mayo de 2025, empleando los términos: “GLP-1 receptor agonists” AND “cardiovascular prevention” AND “non-diabetic patients”. Se incluyeron ensayos clínicos aleatorizados (ECA), estudios de cohorte, metaanálisis y revisiones sistemáticas con ≥10 pacientes no diabéticos. Se excluyeron estudios en pacientes con diabetes, animales o series de caso pequeñas. Se aplicaron herramientas RoB 2.0 y Newcastle–Ottawa Scale para evaluar calidad metodológica. La síntesis se realizó conforme a las directrices PRISMA 2020. Resultados: De 1,234 registros, se incluyeron ocho estudios relevantes. El ensayo SELECT (n = 17,604), un ECA fase III, evaluó semaglutida 2.4 mg/semana en adultos con obesidad y enfermedad cardiovascular sin diagnóstico de diabetes [6]. La intervención redujo el riesgo relativo de MACE en un 20 % comparado con placebo (HR 0.80; IC 95 % 0.72–0.90; p < 0.001). Subanálisis prespecificados mostraron beneficios consistentes en pacientes con enfermedad coronaria crónica y función renal moderadamente reducida [7,8]. No se identificaron otros ECA con GLP-1RA distintos a semaglutida en esta población. Las reacciones adversas más frecuentes fueron gastrointestinales, con una tasa de suspensión mayor en el grupo activo [6]. Conclusión: Semaglutida es el primer GLP-1RA con evidencia robusta que demuestra reducción de eventos cardiovasculares mayores en pacientes sin diabetes con enfermedad cardiovascular establecida. Se requieren ensayos clínicos adicionales con otras moléculas y en prevención primaria para fortalecer las recomendaciones terapéuticas en esta población.
