Publications Rates and Characteristics of Registered Cornea Transplant Trials: An Analysis of Clinicaltrials.gov Leo Meller1, Ibrahim Abboud2, Steven Anderson2, and Natalie Afshari3 1Shiley Eye Institute, San Diego, CA; 2UC Riverside School of Medicine, Riverside, CA; and 3UC San Diego, San Diego, CA. Purpose: To characterize all registered corneal transplant (CT) trials. Methods: Information of all registered CT trials was extracted from Clinicaltrials.gov. Results: A total of 164 CT trials were identified with no time limits with the oldest from 1986, to which the majority were interventional (123, 75%), early phase or uncategorized (139, 84.8%), focused on adults only (142, 86.6%), non-industry sponsored (151, 92.1%). Principal investigators were predominately male (106, 64.6%), based internationally (114, 69.5%), and led by MD only (94, 57.3%). Interventional trials were more likely to be late phase (P = 0.002) and industry sponsored (P = 0.02) compared to observational trials. Among completed trials, 74 (63.2%) were published. International trials and trials focused on adults only were associated with publication (P = 0.003, P < 0.001, respectively). Conclusions: Less than two-thirds of completed CT trials were published in peer reviewed journals. Industry played a more significant role in interventional trials and international trials were associated with publication status. Increasing efforts is needed to improve rates of publication for timely dissemination of the latest advances in CT. “Needle” Keratoplasty Delivery of Endothelial Donor Grafts with a Novel Needle Delivery System Eric Abdullayev, and Arthur Kurz Lions World Vision Institute, Tampa, FL. Purpose: To introduce the first delivery device for the eye bank prepared/preloaded endothelial grafts to be delivered into the recipient's anterior chamber through the puncture. Methods: Donor corneas with healthy endothelium were used. 5 DMEK and 5 UT-DSAEK grafts were prepared loaded in the new delivery device with a needle insertion tip and stored for 5 days in Optisol-GS at 2 to 8 C. Then grafts were unloaded and endothelial viability was evaluated using specular microscopy and a new cell staining method. Images are digitally processed by the “ImageJ” image processing program. The percentage of the damaged cells was calculated. An artificial chamber with mounted donor corneas was used to recreate graft insertion through the puncture. Results: Mean preparation donor endothelial cells density was 2593 ± 89 cells/mm2 and post load - 2655 ± 95 cells/mm2. On the first day following preload, the mean endothelial cell loss was 5%, which remained stable at day 5. Endothelial specular microscopy and staining confirmed viable endothelium. All grafts were inserted through the needle puncture without difficulties. No fluid leaks were observed from the puncture wound. Conclusions: Eye bank-prepared pre-loaded DMEK and UT-DSAEK grafts in the new delivery device with a needle insertion tip survive storage for several days and can be successfully inserted for transplantation through puncture without prior blade incision. Ocular Features of Severe Vitamin Deficiency in Autistic Spectrum Disorder Children in the USA Anas Abuseif, Oliver Beale, and Ken Nischal UPMC Children's Hospital of Pittsburgh, Pittsburgh, PA. Purpose: To describe 3 patients with autistic spectrum disorder (ASD) and peculiar restrictive diet who presented with severe vitamin A deficiency in the USA. Methods: Retrospective chart review. Results: Two (males) presented with blepharospasm, photosensitivity and night-blindness. Only at exam under anesthesia were bilateral corneal melts found in both. Both were treated with Amniotic Membrane Transplant with tarsorrhaphy; one underwent bilateral Gundersen flaps as a secondary procedure. One female presented with severe bilateral punctate epithelial erosions and blepharospasm. All patients had severe xerophthalmia, conjunctival ulceration, and Bitot's spots and received vitamin A intramuscularly along with aggressive lubricating eye drops. Behavioral therapy was needed together with continued oral Vitamin A to improve diet. All patients improved significantly. Conclusions: The possibility of severe vitamin A deficiency should be considered in ASD patients who present with photophobia and blepharospasm. Corneal Densitometry in Congenital Hereditary Endothelial Dystrophy: A Novel Attempt to Re Classify Corneal Clouding Muralidhar Ramappa1, Divya S. Purpose: To objectively grade corneal cloudiness in children with CHED and correlate it with clinical findings. Methods: This retrospective case-controlled study included 55 cloudy corneas from 32 CHED children, compared with 35 clear corneas from 18 age-matched healthy controls. Corneal densitometry (CD) was assessed using the WaveLight Oculyzer II, while central corneal thickness (CCT) was measured using anterior segment OCT. Corneal cloudiness was subjectively assessed via slit-lamp images and correlated with central 4 mm area densitometry. Objective corneal clouding grades were mild (28 eyes), moderate (16 eyes), and severe (5 eyes). Results: Combining clinical and imaging modalities, corneal clouding was graded as mild (52 eyes), moderate (25 eyes), and severe (9 eyes). The median age at the imaging time was 7.6 (5.3–10.1) years, with 58.3% being male. More advanced cases presented higher backscatter (P <0.0001). Mean central 4 mm area densitometry was significantly higher than age-matched healthy controls (P < 0.0001). Corneal grading comparisons showed a significant correlation between slit-lamp findings and densitometry measurements (P < 0.0001). CCT and LogMAR visual acuity were significantly correlated with area densitometry (P < 0.0001 and P = 0.004, respectively). Age at investigation and gender did not correlate with area densitometry (P = 0.32 and P = 0.19). Significant differences were observed for densitometry measurements across 4 annuli (P < 0.0001) and 3 corneal layers (P < 0.0001). Conclusions: A densitometry program is valuable for assessing corneal clouding in CHED patients. Densitometry measurements were significantly correlated with CCT and Log MAR visual acuity, indicating the program's utility in accurately evaluating corneal cloudiness in children with CHED.
