Penyediaan air minum yang aman, layak, dan berkelanjutan merupakan tantangan besar bagi kota-kota berkembang di Indonesia, termasuk Kota Medan. Meskipun target nasional sebesar 100% layanan air minum pada tahun 2030 telah dicanangkan, berbagai kendala struktural dan teknis masih menghambat pencapaiannya. Penelitian ini bertujuan untuk mengidentifikasi potensi sumber daya air baku alternatif, menganalisis tantangan tata kelola dan kebijakan, serta merumuskan strategi penguatan ketahanan penyediaan air di Kota Medan. Metodologi yang digunakan adalah pendekatan deskriptif kualitatif melalui telaah kebijakan, studi dokumen, wawancara pemangku kepentingan, dan skoring multi-kriteria berbasis efektivitas, efisiensi, dan dampak jangka panjang. Temuan utama menunjukkan bahwa Kota Medan masih sangat bergantung pada sumber air baku konvensional seperti Sungai Deli dan Belawan yang mengalami degradasi kualitas dan kuantitas. Potensi air alternatif seperti Sungai Bingei, air hujan, dan air limbah terolah belum dimanfaatkan secara optimal. Sementara itu, ketergantungan kelembagaan terhadap PDAM Tirtanadi sebagai BUMD provinsi menimbulkan kesenjangan koordinasi dan perencanaan di tingkat kota. Hasil evaluasi menunjukkan bahwa pembentukan mekanisme koordinasi lintas daerah untuk pengelolaan sumber air bersama merupakan pilihan kebijakan paling prioritas, diikuti oleh investasi teknologi air alternatif dan program efisiensi air di sektor konsumsi. Konsep-konsep seperti Integrated Urban Water Management (IUWM), Water Sensitive Urban Design (WSUD), dan prinsip Good Water Governance menjadi dasar strategis dalam perumusan rekomendasi. Penelitian ini merekomendasikan pembentukan forum lintas wilayah, peraturan wali kota tentang rencana induk sumber air alternatif, serta skema pembiayaan inovatif melalui KPBU dan dana iklim. Hasil kajian ini diharapkan menjadi referensi bagi pembuat kebijakan dalam merumuskan tata kelola air yang lebih integratif, adaptif, dan berkelanjutan di Kota Medan.
