Résumé La crise des opioïdes, débutée aux États-Unis dans les années 2010, a causé plus de 727 000 décès entre 1999 et 2022, réduisant l’espérance de vie de 0,67 an en 2022. Elle a été amplifiée par Purdue Pharma, qui a commercialisé l’OxyContin dès 1996 en minimisant son risque addictif. Une lettre de 1980 du New England Journal of Medicine a été largement et abusivement citée pour soutenir cette thèse. Purdue a promu agressivement son médicament auprès des médecins généralistes, favorisant une prescription massive. L’évolution de cette crise se divise en trois phases : d’abord une hausse des décès liés aux opioïdes prescrits, puis une augmentation des overdoses d’héroïne dès 2010, et enfin, à partir de 2013, une explosion des décès dus au fentanyl de synthèse. La crise, initialement concentrée chez les populations blanches aisées, touche désormais les populations racisées et défavorisées. Depuis 2015, les restrictions sur les prescriptions d’opioïdes ont entraîné un basculement vers des opioïdes illicites. Si la crise reste majeure aux États-Unis et au Canada, l’Europe, bien que connaissant une augmentation de la consommation d’opioïdes, présente une situation contrastée. En France, la hausse de l’usage de l’oxycodone et du tramadol inquiète les autorités. Pour lutter contre cette crise, des recommandations de la HAS incitent à évaluer le risque de mésusage lors de l’introduction ou du renouvellement, et limiter le recours aux doses élevées. L’OMS déconseille désormais l’usage des opioïdes dans certaines pathologies comme la lombalgie chronique
