Off-label therapeutic potential of topical roflumilast in dermatology: A narrative review

Abstract
Topical roflumilast, a potent phosphodiesterase-4 inhibitor, was recently approved for the treatment of plaque psoriasis, seborrheic dermatitis, and atopic dermatitis. By reducing cyclic adenosine monophosphate degradation, roflumilast downregulates proinflammatory cytokines implicated in a variety of dermatologic diseases. Recently, several case reports have described the off-label use of topical roflumilast in conditions including nail psoriasis, paradoxical psoriasis, palmoplantar pustulosis, erythema annulare centrifugum, chronic nonscarring scalp folliculitis, lichen simplex chronicus, vitiligo, cutaneous lupus erythematosus, and granuloma annulare. In addition, oral roflumilast has shown benefits in case reports on oral lichen planus, hidradenitis suppurativa, and Behçet's disease, supporting a mechanistic rationale for exploring topical roflumilast in these clinical contexts. While further studies are required to elucidate its full therapeutic potential, roflumilast is a welcome addition to current nonsteroidal topical treatments for inflammatory dermatologic disease.
