Évaluation des déterminants sociocognitifs dans l’utilisation de la glace en traumatologie du sport : étude transversale observationnelle sur 540 sportifs

Abstract
L’application de glace dans le contexte d’une pathologie musculosquelettique (ou blessure) aiguë est une pratique largement répandue en traumatologie du sport, bien que son efficacité reste discutée et repose principalement sur des données empiriques ou des études de faible niveau de preuve. Or, les comportements sont influencés par des déterminants sociocognitifs (aussi appelés croyances) tels que les attitudes, les normes sociales perçues et le contrôle comportemental perçu, qui contribuent à la formation de l’intention comportementale. Dans ce contexte, cette étude visait à identifier les déterminants sociocognitifs qui sous-tendent l’utilisation de la glace en traumatologie du sport. Nous avons réalisé une étude transversale observationnelle au moyen d’un questionnaire anonyme en ligne adressé à des sportifs de tous niveaux et francophones. Nous avons mesuré les déterminants sociocognitifs de l’utilisation de la glace à l’aide de 18 questions auxquelles les participants devaient répondre en utilisant des échelles de Likert en 7 points. Nous avons analysé les potentielles corrélations entre ces déterminants et l’utilisation de la glace. L’analyse a porté sur 540 sportifs âgés en moyenne de 28,6 ± 9,7 ans, avec un sexe ratio de 1,1 en faveur des femmes. Les résultats montraient que les croyances attitudinales instrumentales étaient significativement fortement corrélées à l’utilisation de la glace. Les croyances comportementales en lien avec la capacité d’utiliser la glace étaient également significativement modérément corrélées à l’utilisation de la glace. Les croyances normatives injonctives et descriptives étaient significativement modérément associées à l’utilisation de la glace. Nous avons pu constater que les attitudinales instrumentales et comportementales en lien avec l’utilisation de la glace étaient des déterminants pertinents dans l’utilisation de la glace en traumatologie du sport, et pourraient être utilisées pour orienter les pratiques en fonction des preuves scientifiques. The use of ice in the context of acute musculoskeletal injuries is a widespread practice in sports trauma, although its effectiveness remains debated and is based primarily on empirical data or studies with low levels of evidence. Despite this scientific uncertainty, its use remains common among athletes. Behaviors are influenced by sociocognitive determinants (or beliefs), such as attitudes, perceived social norms, and perceived behavioral control, which impact through behavioral intention. In this context, this study aimed to identify the sociocognitive determinants underlying the use of ice in sports trauma. We conducted a cross-sectional observational study using an anonymous online questionnaire addressed to athletes from all sports, all levels, and French-speaking. We collected the various sociocognitive determinants of the use of ice using 18 questions that athletes were asked to answer using a 7-point Likert scales. We analyzed correlations between these determinants and ice use. In total, the analysis included 540 athletes (mean age = 28.6 years, SD = 9.7, with a gender ratio of 1.1 in favor of women). The results showed that instrumental attitudinal beliefs were significatively strongly correlated with ice use. Behavioral beliefs related to the ability to use ice were also significantly moderately correlated with ice use. Injunctive and descriptive normative beliefs were significatively moderately associated with ice use. Our findings showed that instrumental and behavioral attitudes related to the use of ice were relevant determinants in the use of ice in sports trauma, and could be used to guide practices based on scientific evidence.
