Abstract
During a diaper change, a previously healthy six-week-old boy was noted to have new left leg swelling involving the foot, ankle, and calf. Over the next two days, the swelling remained unchanged. There was no redness, warmth, or bruising and no history of trauma. He was otherwise well without changes to his feeding or the functioning of his leg. He was brought to the hospital because of the persistently swollen limb. On examination in the Emergency Department, he was afebrile, vitally stable, and non-dysmorphic. Weight was 6.0 kg (97th percentile). His left leg measured 17 cm at the widest point in the calf, and the right measured 15 cm. There was no pitting edema, erythema, tenderness, warmth, bruising, or nodules. The remainder of the general physical exam was normal (Figure 1). ... He was born term via vaginal delivery without complications and required one day of phototherapy. Family history was negative for malignancy, clotting or bleeding disorders.
