Abstract
Un groupe de quatre voyageurs canadiens en bonne santé a visité un cénote dans la péninsule du Yucatán en avril 2024 et a développé par la suite une histoplasmose symptomatique.Le diagnostic a été posé pendant la phase aiguë grâce à un test antigénique urinaire positif à Histoplasma dans trois des cas.Deux d'entre eux ont présenté des formes graves et ont été traités à l'itraconazole, dont un enfant de trois ans atteint d'une maladie disséminée.La sensibilité des différentes modalités de diagnostic dépend du moment et de la gravité de la maladie, l'antigène urinaire d'Histoplasma étant le plus sensible au début de l'infection, la sérologie se convertissant 4 à 8 semaines après l'exposition et les cultures étant généralement peu sensibles.Le traitement dépend des manifestations de la maladie et de l'état immunologique de l'hôte.De nombreux patients présentent un syndrome grippal relativement bénin et spontanément résolutif, et le diagnostic peut passer inaperçu.Compte tenu du nombre de touristes canadiens qui se rendent dans la péninsule du Yucatán et de la popularité de la visite des cénotes, il convient de sensibiliser au risque d'histoplasmose associé à cette exposition.
