A contusão encefálica é uma forma comum e grave de lesão cerebral traumática (LCT), frequentemente associada a disfunções neurológicas persistentes. O manejo cirúrgico tem passado por importantes transformações ao longo das últimas décadas, com o avanço das técnicas minimamente invasivas e da monitorização multimodal. Este artigo tem como objetivo realizar uma revisão sistemática da literatura sobre o tratamento neurocirúrgico da contusão encefálica, destacando a evolução dos métodos terapêuticos e seu impacto sobre a neuroplasticidade pós-trauma. A pesquisa foi conduzida nas bases de dados SciELO, LILACS e BVS, selecionando artigos brasileiros publicados nos últimos 15 anos. Os resultados indicam que intervenções precoces, aliadas a estratégias cirúrgicas menos agressivas e à integração com terapias de reabilitação, favorecem a recuperação funcional do tecido cerebral, promovendo melhor reorganização sináptica. A compreensão da neuroplasticidade como processo dinâmico influenciado por intervenções cirúrgicas adequadas pode redefinir paradigmas no tratamento da LCT.
