Abstract
Le partenariat intersectoriel optimise la qualité des services éducatifs et favorise une transition harmonieuse vers l’école (Moreau et al., 2005). Cet article examine la Démarche Mon Portrait, une initiative régionale visant à renforcer la collaboration entre les réseaux de la petite enfance, de la santé et de l’éducation. En développant une vision commune et en respectant les contextes de chaque réseau, cette approche promeut la continuité éducative, le bienêtre et les apprentissages des enfants. L’article détaille cette démarche et propose des pistes pour inspirer d’autres initiatives similaires.
