A New Technique for Mechanical Thrombectomy in Patients With Acute Pulmonary Embolism: A Case Series

Abstract
Acute PE is a serious illness that has a high mortality. Systemic anticoagulation, systemic thrombolysis, catheter-directed thrombolysis, and mechanical thrombectomy are the four main types of therapies used today for PE. The innovative "smart" catheter tubing system of the Penumbra Indigo System (Penumbra Inc., Alameda, CA) is aKeyon thrombectomy. Despite the Penumbra indigo system's effectiveness and thorough research, many governments-especially those in developing countries-are unable to use it because of its high cost and shortage. Here, we present a novel, homemade, inexpensive, and simple Penumbra-like technique (Marwan-Ayman Pulmonary Embolectomy "MAPE" technique) for thrombus aspiration in this case series. We substituted a Judkin right catheter 7 French for the Penumbra catheter, a three-lobed snare for the separator and the standard Y connector to connect to Amico vacuum suction device (Ontario, Canada) which substitute Penumbra engine and is adjusted at -29 mmhg. We used this procedure in four cases with excellent outcomes.
