Abstract
Periprosthetic femoral fractures around hemiarthroplasty implants in elderly patients pose significant challenges. We report an 87-year-old male with a Vancouver type B1 periprosthetic femoral shaft fracture around an Austin Moore prosthesis, successfully managed with open reduction and internal fixation (ORIF) using a distal femoral locking plate, lag screws, and cerclage wiring. Radiographic union was achieved, and the patient returned to pre-injury mobility status. This case highlights that ORIF with locking plate constructs remains a safe and effective option for B1 fractures, even in very elderly patients, provided stem stability is confirmed.
