Abstract
A 6-week-old term female presented for routine care; the community paediatrician noted significant pallor on initial assessment. There was no history of jaundice or bleeding. Physical exam demonstrated tachycardia (180 beats per minute) with normal blood pressure, and several hyperpigmented maculopapular lesions on her face and extremities. The infant was directed towards the paediatric emergency department for further evaluation. Initial investigations demonstrated severe normocytic anemia (hemoglobin 43 g/L, mean corpuscular volume 95 fL), inappropriate reticulocytosis (absolute reticulocytes 70.6 × 109/L), and normal platelets and white blood cells. History revealed a ‘blueberry muffin rash’ (Figure 1a), characterized by multiple 1–8 mm purplish raised and flat lesions on her face, torso, and extremities. Evaluation initially included a prolonged partial thromboplastin time at 43 seconds (normal 26–38 seconds), mildly elevated aspartate aminotransferase 134 U/L (normal <115 U/L) with otherwise normal liver enzymes and function, and normal hemoglobin (154 g/L). Screen for congenital infections was negative. The skin lesions spontaneously regressed, and she was discharged home with no symptoms until presentation at 6 weeks of age.
