Abstract
Retensio plasenta merupakan salah satu penyebab utama perdarahan postpartum yang dapat berujung pada syok hipovolemik dan kematian maternal apabila tidak segera ditangani secara tepat. Kami melaporkan kasus seorang wanita berusia 36 tahun, P3A0, yang dirujuk ke Instalasi Gawat Darurat RSUD Ciawi dari praktik bidan. Pasien menjalani persalinan spontan pada usia kehamilan 41 minggu di fasilitas bidan, namun dua jam pascapersalinan mengalami perdarahan hebat dengan plasenta yang belum lahir. Pemeriksaan fisik menunjukkan konjungtiva anemis, tekanan darah 100/70 mmHg, dan frekuensi nadi 100 kali per menit. Pemeriksaan penunjang menunjukkan kadar hemoglobin sebesar 8,5 g/dL serta hasil ultrasonografi yang mengidentifikasi adanya sisa plasenta dalam kavum uteri. Pasien tidak memiliki riwayat operasi sesar sebelumnya. Tindakan segera dilakukan untuk menghentikan perdarahan dan mengevakuasi jaringan plasenta yang tersisa. Kasus ini menekankan pentingnya deteksi dini dan penanganan cepat pada retensio plasenta guna mencegah komplikasi lanjut yang berpotensi fatal.
