Abstract
Проаналізовано роль предиктивної медицини в становленні майбутньої охорони здоров’я. Підкреслено, що серед різних методологій таких, як геноміка, протеоміка та цитоміка найбільш фундаментальний спосіб прогнозування майбутніх захворювань базується на генетиці. Розглянуто етичні проблеми, пов’язані з особистою відповідальністю, що викликають епігенетичні тести прогнозування ризику розвитку захворювань. Поява нових методів лікування з предиктивними біомаркерами для визначення пацієнтів, які найімовірніше чи менш ймовірно отримають користь, ускладнює розроблення лікарських засобів. Хоча для багатьох нових онкологічних препаратів це єдиний науково обґрунтований підхід, що має збільшити шанси на успіх. З’ясовано, що предиктивна медицина може призвести до більшої узгодженості результатів у випробуваннях і має очевидні переваги для зменшення кількості пацієнтів, які в кінцевому підсумку отримують дорогі ліки, що наражає їх на ризик побічних ефектів, але не приносить користі. Предиктивна медицина має велике потенційне значення для контролю суспільних витрат на охорону здоров’я. Розроблення методів лікування за допомогою прогностичних біомаркерів вимагає серйозних змін у стандартних парадигмах планування та аналізу клінічних випробувань. Використання методів предиктивної медицини потребує перегляду низки дизайнів клінічних випробувань для спільного створення методів лікування та прогностичних біомаркерів. Відповідно нові підходи до планування можуть стати основою для нового покоління предиктивних клінічних випробувань, що надають надійну індивідуалізовану інформацію.
