Non-immune hydrops fetalis associated with congenital bands and intestinal obstruction: A case report

Abstract
Introduction: Non-immune hydrops fetalis can develop because of increased fluid in the interstitium and may be secondary to high plasma oncotic pressure caused by intestinal obstruction. Congenital bands are an uncommon cause of intestinal obstruction in the newborn. Neonatal intestinal obstruction should be considered as a rare underlying cause of non-immune hydrops when other, more common causes have been excluded. Case Presentation: A male infant was delivered at 29 weeks, 4 days of completed gestational age after an antenatal ultrasound demonstrated cardiomegaly, pericardial effusion, and multiple dilated echogenic fluid-filled bowel loops. Postnatal examination was remarkable for anasarca with peripheral edema and a distended abdomen. Trophic feeds were commenced on day 6 of life; however, rectal bleeding was seen soon after. Multiple imaging investigations were performed, with a contrast enema showing a discrepancy between the caliber of the large and small intestines. Exploratory laparotomy identified a thick congenital vascular adhesion band causing a partial obstruction. The infant had an uncomplicated post-operative course. Conclusion: Congenital vascular bands should remain as a differential diagnosis for cases in which radiological imaging has not identified characteristic findings of other causes of newborn intestinal obstruction.
