Abstract

La semaglutida (Ozempic®), un agonista del receptor GLP-1 ampliamente utilizado en diabetes mellitus tipo 2 y obesidad, ha demostrado beneficios metabólicos y cardiovasculares comprobados. Sin embargo, evidencia reciente sugiere una posible asociación con neuropatía óptica isquémica anterior no arterítica (NAION), un evento adverso ocular grave que puede ocasionar pérdida visual irreversible. Se realizó una revisión sistemática siguiendo PRISMA 2020 en PubMed, Scopus y Web of Science (2017–2025), incluyendo ensayos clínicos, estudios observacionales, reportes de casos y farmacovigilancia. La calidad metodológica se evaluó con RoB 2.0, Newcastle-Ottawa y JBI. Se identificaron 18 estudios. Las cohortes poblacionales y multinacionales reportaron un aumento del riesgo de NAION en usuarios de semaglutida, con HR de 2.0–3.2, OR de 3–5 y RR de 4.5. Reportes de casos describieron pérdida visual súbita tras iniciar el fármaco. Análisis de la base FAERS mostraron desproporcionalidad significativa en notificaciones, y un metaanálisis reciente de 29 estudios confirmó una señal consistente de seguridad ocular. No obstante, la incidencia absoluta es baja (<0,1%) y la heterogeneidad metodológica limitó la integración estadística. La asociación entre semaglutida y NAION es estadísticamente consistente, pero no concluyente en cuanto a causalidad. Se recomienda monitoreo oftalmológico en pacientes con factores de riesgo y fortalecer la farmacovigilancia. Se requieren estudios prospectivos y multicéntricos para definir la magnitud real del riesgo y guiar recomendaciones clínicas.
