Abstract
Abstract The behavioral variant of frontotemporal dementia (bvFTD) is characterized by atrophy in the frontal and anterior temporal lobes and affects behavior, personality, and executive control. Diagnosing bvFTD is challenging due to overlapping symptoms with other neurodegenerative diseases, despite established diagnostic criteria focusing on behavioral changes. Recent studies highlight the importance of speech and language symptoms in bvFTD, which are often underrecognized yet crucial for early detection, differential diagnosis, and disease management. This chapter reviews speech and language symptoms in bvFTD and the role of language tests in distinguishing bvFTD from other disorders such as primary progressive aphasia variants, Alzheimer’s disease, psychiatric disorders, amyotrophic lateral sclerosis, and corticobasal syndrome/progressive supranuclear palsy. The chapter also examines longitudinal changes in speech and language with disease progression and early detection of linguistic symptoms in preclinical FTD caused by genetic mutations.
