Closing the Practice Gap in Secondary Prevention After Acute Coronary Syndromes: a Provincial Implementation Pathway

Abstract
Adults with atherosclerotic cardiovascular disease (ASCVD) discharged following an acute coronary syndrome (ACS) require evidence-based secondary prevention to mitigate the risk of recurrent vascular events, long-term disability, and mortality. While guideline-recommended secondary prevention strategies are effective, their implementation is often inconsistent and fragmented. To address this, the Canadian Cardiovascular Society has developed a stakeholder-informed Secondary Prevention Pathway (SPP) aimed at standardizing care and improving uptake of evidence-based recommendations post-ACS. This evidence-based, multidisciplinary pathway aims to enhance adherence to guideline-directed care and optimize patient outcomes after ACS. This paper describes development and initial evaluation of the SPP.
