Мета роботи. Оцінити ефективність комбінованого застосування АСІТ та вітаміну D у пацієнтів із сезонним алергічним ринітом, сенсибілізованих до пилку амброзії (Ambrosia artemisiifolia). Матеріали і методи. У дослідження залучили 125 пацієнтів із сезонним алергічним ринітом, сенсибілізованих до пилку Ambrosia artemisiifolia, розподілених на дві порівнювані групи: контрольну ($n \approx 55$) та інтервенційну ($n \approx 75$). Оцінювання здійснювалося у трьох точках: до початку лікування ($T_0$), через 12 місяців від початку терапії ($T_1$) та через 24 місяці спостереження ($T_2$). Клінічні індекси включали вираженість симптомів АР за візуальною аналоговою шкалою, частоту/потребу в топічних ГКС та частоту/потребу в антигістамінних препаратах. Імунологічний профіль визначали за рівнями загального IgE та специфічного IgE до амброзії – Amb a1, цитокінів IFN-$\gamma$, IL-4, IL-10, IL-13 та TGF-$\beta$. Проводився аналіз головних компонент (PCA) та ієрархічний кластерний аналіз. Результати дослідження. Виявлено, що PC1 відображає вісь алергічного запалення та Th2 опосередкованої сенсибілізації, оскільки найбільший внесок робили показники tIgE, sIgE Amb a1, IL-4 та IL-13. PC2 істотно корелювала з вираженістю симптомів (VAS), потребою в антигістамінних препаратах та використанням назальних ГКС, що відповідає осі клінічної активності та медикаментозної залежності. PC3 мала найбільші навантаження на IL-10 та TGF-$\beta$ і відображала регуляторну/толерантну імунну відповідь, потенційно модульовану вітаміном D. Додавання вітаміну D зумовлює зсув профілю в бік меншої Th2 активності (рух уздовж PC1 до нижчих IgE/IL-4/IL-13) та посилення регуляторних механізмів (проєкція на PC3). Під час кластерного аналізу було виділено три фенотипи. Пацієнти з регуляторним фенотипом (кластер 2) ймовірно найбільше виграють від комбінованого лікування (АСІТ + вітамін D), тоді як пацієнти з вираженою Th2 активацією (кластер 3) потребують розширених стратегій контролю запалення. Висновки. Результати дослідження розширюють розуміння імуномодулюючої ролі вітаміну D у поєднанні з АСІТ та демонструють перспективи впровадження комбінованих терапевтичних підходів у клінічну практику лікування пацієнтів із алергічним ринітом, сенсибілізованих до пилку амброзії.
