Abstract

fetched live from OpenAlex

Forward: On May 29, 2025 in Quebec City, Dr. Jeremy Friedman was awarded the inaugural Canadian Paediatric Society Distinguished Hospital Paediatrician Award by the Section of Hospital Paediatrics. Although he could not attend in person, Dr. Friedman shared this written acceptance speech, which I had the honour of reading aloud to section members that evening in the intimate dining room of a restaurant in Old Québec. Sanjay Mahant I wanted to sincerely thank all those who were involved in awarding me with this honour. It is very meaningful to me as my entire career is based on hospital paediatrics. I had hoped to be in Quebec City at the Canadian Paediatric Society (CPS) meeting giving one of my favourite talks of the year with Sanjay, which we have done for quite a few years now on the theme of ‘Choosing Wisely’. Instead, I send you this note from my hospital room at Princess Margaret where I am slowly recovering after my second allogeneic BMT since February 2022. I am being treated for a rare bone marrow malignancy, known as myelofibrosis, which was discovered incidentally by a radiologist on an MRI scan for an old running injury. The irony of the Choosing Wisely guy being diagnosed based on an incidental finding is not lost on me! Anyways, after a lifetime of great health and 35 years at the bedside I am now experiencing what it is like when the doctor becomes the patient. While I have learned much through this experience that will make me a better physician and more mindful person, I would not recommend it!

Not calibrated prevalence, not ground truth. Human validation pending. The Gemma side is a direct model label for every work in the frame, read from the title-only record. The Codex side is a classifier learned from the 10,348 direct Codex labels and calibrated to design-weighted sample rates; fields without enough sample support carry no Codex call. Candidate is the union of the two sides; consensus is their intersection. These outputs are machine_predicted_unvalidated and are not human labels.

Machine scores (provisional)

The two teacher heads of the student model, read on this work. A score orders the frame for review; it never asserts a category, and the validation status ships verbatim with every row.

Baseline scores from an immature model (maturity gate not passed, 7 training rounds). Scores rank; they never assert a category.
