Abstract

La cirugía plástica y ortopédica pediátrica requiere soluciones específicas para abordar defectos congénitos, traumáticos o adquiridos en un organismo en crecimiento. En este contexto, los biomateriales reabsorbibles y los bioestimuladores tisulares han emergido como alternativas prometedoras para optimizar la regeneración ósea y de tejidos blandos, reduciendo complicaciones y evitando reintervenciones.Metodología: Se realizó una revisión sistemática siguiendo la declaración PRISMA 2020. Se consultaron PubMed, Scopus y Web of Science, incluyendo estudios publicados entre 2000 y 2025. Se consideraron elegibles ensayos clínicos, cohortes y series de casos con ≥10 pacientes pediátricos. Se evaluó la calidad mediante RoB-2 y Newcastle–Ottawa Scale. Resultados: De 1,246 registros identificados, se incluyeron 42 estudios. Los implantes reabsorbibles, principalmente polímeros biodegradables y nanocompuestos, mostraron tasas de consolidación ósea superiores al 90%, con baja incidencia de complicaciones leves como inflamación local. En cirugía plástica craneofacial, matrices colágenas e injertos basados en hidroxiapatita demostraron remodelación progresiva y buenos resultados estéticos, aunque con limitaciones de tamaño muestral. En cuanto a bioestimuladores, factores de crecimiento (BMP-2, PDGF) y matrices bioactivas evidenciaron mayor densidad ósea, reducción del tiempo de cicatrización y menor incidencia de complicaciones infecciosas. No se reportaron efectos adversos graves ni interferencias con el crecimiento óseo en seguimientos de hasta 5 años. Discusión: La evidencia disponible apoya la seguridad y eficacia de biomateriales reabsorbibles en ortopedia pediátrica, y sugiere beneficios en reconstrucción plástica craneofacial. Los bioestimuladores representan un campo emergente con gran potencial, aunque requieren ensayos clínicos robustos y seguimientos a largo plazo. Persisten limitaciones metodológicas, como heterogeneidad de protocolos y escasez de estudios multicéntricos. Conclusión: Los biomateriales y bioestimuladores constituyen herramientas innovadoras en la cirugía pediátrica, con capacidad de transformar la práctica clínica. Futuras investigaciones deben enfocarse en el desarrollo de biomateriales híbridos y en la evaluación de impacto a largo plazo sobre el crecimiento y desarrollo infantil.
