Abstract
Đặt vấn đề: Gãy đầu dưới xương quay ở người cao tuổi là chấn thương thường gặp và có thể dẫn đến suy giảm chức năng vận động và giảm khả năng độc lập trong sinh hoạt hàng ngày. Nghiên cứu này được thực hiện nhằm đánh giá hiệu quả của một chương trình hoạt động trị liệu chuyên biệt, lấy người bệnh làm trung tâm trong việc phục hồi chức năng cho nhóm bệnh nhân này. Phương pháp: Nghiên cứu tiến cứu được thực hiện trên 30 bệnh nhân từ 60 tuổi trở lên ngay sau khi kết thúc điều trị bảo tồn gãy đầu dưới xương quay bằng bó bột. Bệnh nhân tham gia một chương trình hoạt động trị liệu tại nhà kéo dài 8 tuần. Các chỉ số về tầm vận động khớp cổ tay, lực nắm bàn tay, mức độ đau (VAS - Visual Analog Scale), chức năng chi trên (QuickDASH- Quick Disability of the Arm, Shoulder and Hand) và khả năng thực hiện, mức độ hài lòng trong sinh hoạt hàng ngày (COPM – Canadian Occupational Performance Measure) được lượng giá trước và sau can thiệp. Kết quả: Sau 8 tuần can thiệp, tất cả các chỉ số lượng giá đều cho thấy sự cải thiện có ý nghĩa thống kê (p < 0.05). Tầm vận động và lực nắm tăng lên đáng kể. Mức độ đau khi vận động giảm mạnh. Điểm chức năng QuickDASH trung bình giảm từ 56.33 xuống 22.33. Điểm thực hiện và hài lòng trung bình theo thang đo COPM cũng tăng lên rõ rệt, cho thấy sự cải thiện lớn về khả năng độc lập trong các hoạt động có ý nghĩa với người bệnh. Kết luận: Chương trình hoạt động trị liệu 8 tuần đã chứng minh hiệu quả rõ rệt trong việc phục hồi chức năng và nâng cao khả năng độc lập trong sinh hoạt hàng ngày cho bệnh nhân cao tuổi sau gãy đầu dưới xương quay.
