To assess the efficacy of local radical excision combined with ALA-photodynamic therapy (ALA-PDT) in moderate-to-severe hidradenitis suppurativa (HS). Retrospective analysis of 11 moderate-to-severe HS patients treated with Local radical excision combined with ALA-photodynamic therapy. All patients achieved clinical response. Two had focal recurrences at 6 and 9 months (no recurrence after reintervention for 15 months); four had minor recurrences (6–17 months, controlled topically); five remained recurrence-free (6–24 months). DLQI improved from 21.18±3.68 to 6.18±3.89. Vancouver Scar Scale scores (2–8) were acceptable with no functional impairment. Local radical excision combined with ALA-photodynamic therapy is a safe and effective treatment for moderate to severe hidradenitis suppurativa.This approach accelerates wound healing, reduces recurrence, and minimizes scarring.
