Fluctuating Atrial Thrombus and Failure of Direct Oral Anticoagulation in Severe Rheumatic Mitral Stenosis

Abstract
Atrial fibrillation and stroke are common complications of rheumatic mitral stenosis (MS) and require anticoagulation. Current guidelines recommend vitamin K antagonists (VKA) as the preferred choice for stroke prevention and management of intra-cardiac thrombus 1 . Unlike direct oral anticoagulants (DOAC), VKA require routine bloodwork for safety and efficacy monitoring. Herein, we describe the case of a migrant seeking refugee status with rheumatic MS, atrial fibrillation and intra-cardiac thrombus who could not receive a VKA, due to socioeconomic barriers. We highlight the echocardiographic thrombus disease course from initial diagnosis, then DOAC failure with thrombus growth, and finally improvement with VKA use.
