Abstract
Las fracturas periprotésicas representan una de las complicaciones más devastadoras y complejas en el campo de la cirugía reconstructiva articular. Con el aumento exponencial de las artroplastias primarias y de revisión a nivel mundial, la incidencia de estas lesiones ha alcanzado cifras alarmantes, convirtiéndose en una epidemia silenciosa en la traumatología geriátrica. Este artículo ofrece una revisión exhaustiva y actualizada sobre la epidemiología, biomecánica y manejo integral de las fracturas alrededor de implantes de cadera y rodilla. Se analiza en profundidad la aplicación clínica del Sistema de Clasificación Unificado (UCS) frente a las clasificaciones tradicionales de Vancouver y Lewis-Rorabeck. Se discuten las estrategias terapéuticas avanzadas, desde la osteosíntesis mínimamente invasiva con placas de estabilidad angular y el uso de puntales corticales alogénicos, hasta las complejas cirugías de recambio protésico con vástagos modulares, conos de metal trabecular y megaprótesis tumorales. Asimismo, se abordan las innovaciones tecnológicas recientes, incluyendo la planificación tridimensional, la impresión de guías personalizadas y el uso de nuevos materiales como las placas de fibra de carbono. El éxito del tratamiento depende de un enfoque multidisciplinar, una planificación preoperatoria meticulosa y la correcta ejecución técnica para restaurar la función y garantizar la supervivencia del implante y del paciente.
