Abstract
Zusammenfassung Eine 26-jährige Patientin stellte sich nach einer Brasilienreise mit rezidivierenden blutigen Diarrhöen, Abdominalschmerzen und subfebrilen Temperaturen vor. Zunächst erfolgte der Nachweis von Entamoeba histolytica und Giardia lamblia im Stuhl, woraufhin eine Therapie mit Metronidazol und Paromomycin durchgeführt wurde. Trotz initialer Besserung persistierten die Symptome. Die wiederholte Stuhldiagnostik ergab keinen Erregernachweis mehr. In der Koloskopie fand sich eine kontinuierliche, schwere Pancolitis (Montreal-Klassifikation E3). Die Histologie bestätigte eine chronisch-aktive Kolitis ohne spezifischen Infektionsnachweis. Unter systemischer Steroidtherapie, Mesalazin sowie Substitution von Eisen, Folsäure und Vitamin D kam es zu einer deutlichen klinischen Besserung. Der Fall verdeutlicht die diagnostischen Herausforderungen bei anhaltenden Diarrhöen nach Tropenaufenthalten und die Notwendigkeit frühzeitiger endoskopisch-histologischer Abklärung.
