A fractal-based transmitting conformal periodic surface is proposed. A wide-band transmitting behavior with a single-layer periodic surface is achieved. The periodic surface is symmetric to ensure polarization-insensitive behavior. A −10 dB passband of 100% in the X-band with a maximum insertion loss of 0.3 dB is achieved for normal incidence. For an incidence angle of 45°, a-10 dB bandwidth of <tex xmlns:mml="http://www.w3.org/1998/Math/MathML" xmlns:xlink="http://www.w3.org/1999/xlink">$3 \text{GHz}(75 \%)$</tex> is achieved from 8.5 to 11.5 GHz with a maximum insertion loss of 0.9 dB. The designed periodic surface is then conformed to planar, cylindrical, and hemispherical shapes. A planar periodic surface achieves a 100% bandwidth with an insertion loss of <tex xmlns:mml="http://www.w3.org/1998/Math/MathML" xmlns:xlink="http://www.w3.org/1999/xlink">$\leq 0.5 \text{dB}$</tex>. For the cylindrical surface, 100% bandwidth is achieved with an insertion loss of <tex xmlns:mml="http://www.w3.org/1998/Math/MathML" xmlns:xlink="http://www.w3.org/1999/xlink">$\leq 3.5 \text{dB}$</tex>. For the hemispherical surface, 100% bandwidth is achieved with an insertion loss of <tex xmlns:mml="http://www.w3.org/1998/Math/MathML" xmlns:xlink="http://www.w3.org/1999/xlink">$\leq 2.5 \text{dB}$</tex>.
