Classifying histological images using cell counting estimators

Abstract
Oral epithelial dysplasia (OED) pose a significant clinical challenge due to their unpredictable progression to oral squamous cell carcinoma. Current histopathological grading methods for OED suffer from limited predictive accuracy and inter-observer variability. In this study, we propose a two-phase digital pathology pipeline that leverages lymphocyte density as a novel digital biomarker for malignancy risk assessment in hematoxylin and eosin stained whole slide images. Using a longitudinal dataset of 200 cases, we first estimate lymphocyte counts through clustering and image-derived features, then apply machine learning-based regression models to generate cell density metrics. These metrics are subsequently used to train classifiers that predict malignant transformation in regions of interest. The best performance, achieved by combining a Gaussian Process Regressor with a Multilayer Perceptron classifier, yielded 83% accuracy and perfect precision. This approach demonstrates the potential of automated lymphocyte quantification in enhancing the objectivity and reproducibility of OED risk stratification.
