Abstract
Đặt vấn đề: Gãy xương gót là tổn thương thường gặp trong gãy xương vùng cổ chân, có 70% các trường hợp gãy xương gót là gãy phạm khớp. Trước đây gãy xương gót thường được điều trị bảo tồn nhưng với sự tiến bộ của các phương pháp chẩn đoán hình ảnh cho phép khảo sát rõ hơn về hình ảnh học tổn thương thì phẫu thuật xương gót được áp dụng nhiều hơn, với lợi thế kiểm soát ổ gãy và nắn chỉnh dễ dàng ứng dụng các loại nẹp vít khoá thiết kế theo giải phẫu đã cải thiện kết quả phẫu thuật kết hợp xương gót. Mục tiêu nghiên cứu: Đánh giá kết quả bước đầu điều trị gãy xương gót bằng phương pháp kết hợp xương nẹp vít khóa tại Bệnh viện Đa khoa Thống Nhất tỉnh Đồng Nai. Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang tiến cứu hồi cứu trên 39 bệnh nhân gãy xương gót đã được phẫu thuật kết hợp xương bằng nẹp vít khoá, theo dõi sau 6 tháng sau phẫu thuật. Kết quả: 39 bệnh nhân có tuổi trung bình là 42.6 ± 11.1 tuổi, nam có 36 chiếm 92.3%, nữ có 3 chiểm 7.7%; Phân loại 39 ổ gãy: 11 ổ gãy loại II (28.2%), 24 ổ gãy loại III (61.5%), 4 ổ gãy loại IV (10.3%); Sau mổ thấy góc Boehler được phục hồi (250 - 450) ở 28 bàn chân chiếm 71.8%. Kết luận: Gãy lún xương gót là một trong những tổn thương nặng thường gặp ở tuổi lao động. Việc điều trị nếu không hoàn thiện sẽ để lại nhiều di chứng, ảnh hưởng đến chất lượng cuộc sống và khả năng lao động sau này của bệnh nhân và cũng là gánh nặng chi phí của xã hội.
