Abstract
Awake surgery for supra-tentorial brain tumors has become increasingly common, where it facilitates more extensive and safer resections. To our knowledge, there have been no reported cases of awake surgery for resecting intrinsic tumors in the posterior fossa, despite the involvement of critical functional areas. We present the case of a patient who underwent awake surgery for an extra-ventricular cerebellar neurocytoma involving the middle cerebellar peduncle. The patient experienced no neurological deterioration, and post-operative imaging revealed no residual tumor. At the 3-year follow-up, MRI showed no recurrence. This case opens new avenues for considering awake craniotomies in similar contexts.
