Reoperation in Two Patients with SJM Toronto SPV and Medtronic Freestyle Stentless Bioprosthetic Valves

Abstract
高齢者の大動脈弁狭窄症など生体弁を用いた弁置換術が適応となる疾患に対し，従来のステント付き弁に代わり，耐久性および有効弁口面積といった点で優れるステントレス弁の使用が近年報告されている．今回われわれは2種類の代表的なステントレス弁，SJM-Toronto SPVとMedtronic Freestyle valveの再手術例を経験した．SJM-Toronto SPVでは自己組織との癒着・石灰化が著明で移植弁を可及的に摘出するにとどまり，結果的に小さいサイズの機械弁による再置換を余儀なくされた．一方，Medtronic Freestyle valveは癒着が軽度で，移植弁自体を容易に摘出することができ自己組織の温存が図れたことで，ステントレス弁による再置換が可能となった．いずれのステントレス弁も優れた10年前後の長期遠隔成績が報告されているが，近年は再手術症例も認められ始めており，より慎重な経過観察が必要である．
