Young people with epilepsy face unique challenges during the transition from paediatric to adult care, including seizure management, comorbidities such as intellectual disability and autism spectrum disorder, and psychosocial impacts. Existing transfer templates are often lengthy and not tailored to epilepsy. A concise, epilepsy-specific template could improve care continuity and reduce information loss. This work is part of the TÉCUM program (Transition en Épilepsie Complexe de l’Université de Montréal), which supports youth with epilepsy and their caregivers.
