Introducción: La enfermedad de Alzheimer (EA) es la demencia más prevalente a \nnivel mundial y, recientemente, se ha asociado a la periodontitis, enfermedad \ncrónica oral producida por disbiosis bacteriana. Ciertas bacterias orales se han \ndetectado en cerebro de ratones y humanos y asociado a neuroinflamación. Sin \nembargo, aún no se esclarece la ruta de acceso de estos microorganismos al \ncerebro. Esta revisión analizará posibles vías por las que Porphyromonas gingivalis \ny Aggregatibacter actinomycetemcomitans —y sus factores de virulencia— migran \nhacia el cerebro, por su protagonismo en la periodontitis. \nMetodología: Este estudio se realizó bajo las directrices de la Declaración de Ítems \nPreferidos para Reportes para Revisiones Sistemáticas y Metaanálisis (PRISMA) \ndel 2020. Se definió Población: personas con EA y periodontitis o animales con \nperiodontitis experimental; Intervención: bacterias P. gingivalis y/o A. \nactinomycetemcomitans; Comparación: personas y animales que no padecen EA \ny/o periodontitis y Outcome o Resultado: detección de estas bacterias, o sus \nproductos, en el cerebro de los humanos o animales, explicitando vía de ingreso. \nLa búsqueda se realizó en MEDLINE-PubMed, WEB OF SCIENCE, LILACS y \nSCOPUS. Los estudios fueron seleccionados por dos revisores independientes, y \nconfirmados por un tercero. Se evaluó el riesgo de sesgo de los estudios con las \nherramientas Newcastle-Ottawa y SYRCLE. \nResultados: se encontraron 668 artículos y —según criterios de elegibilidad— se \nseleccionaron 11 estudios originales (realizados en modelos animales de \nperiodontitis y observacionales en humanos) con riesgo de sesgo moderado. Las \ninvestigaciones detectaron P. gingivalis en cerebro, suero, plasma, líquido cerebroespinal o saliva de los sujetos, y mencionaron que ingresó a través de la barrera \nhematoencefálica (BHE) comprometida. Además, detectaron vesículas de \nmembrana externa (VME) de A. actinomycetemcomitans en el cerebro de ratones, \nque cruzaron la BHE, causando neuroinflamación. \nConclusiones: La presencia de bacterias orales fue comprobada en cerebros \nanimales con periodontitis experimental y de humanos, y se postuló que la posible \nvía de entrada es la BHE comprometida.
