As part of The Ottawa Hospital (TOH) stroke pathway, all stroke patients are provided with the Heart and Stroke Foundation booklet u201cYour Stroke Journey: A guide for people living with strokeu201d. In an effort to comply with Canadian Stroke Best Practice Recommendations (CSBPR) which states: u201cAll information intended for patient-use should be available in aphasia-friendly formatsu201d...u2026members of Champlain Regional Stroke Network collaborated on the development of a bilingual, aphasia-friendly tool destined for patients and their families.
