Intravascular Brachytherapy for the Management of Recurrent Drug-Eluting In-Stent Restenosis: A Detailed Case Report with Comprehensive Review and Imaging Insights

Abstract
In-stent restenosis (ISR) following drug-eluting stent (DES) implantation remains a significant clinical challenge, particularly in cases of recurrent ISR resistant to conventional therapies. Intravascular brachytherapy (IVBT) has re-emerged as a highly effective treatment for DES-ISR by delivering targeted radiation to inhibit neointimal hyperplasia. This case report describes a 62-year-old male with recurrent DES-ISR in the left anterior descending artery (LAD), successfully treated with beta-radiation IVBT. We provide an in-depth account of the patient’s clinical presentation, diagnostic evaluation, procedural techniques, imaging findings, and clinical outcomes. Intravascular ultrasound (IVUS) and optical coherence tomography (OCT) were pivotal in guiding the procedure and assessing outcomes. Six illustrative figures are included to highlight angiographic and intravascular imaging findings. A comprehensive review of IVBT’s role in DESISR, supported by 32 references in Vancouver style, discusses its mechanisms, efficacy, safety, and future directions.
