Cambios en una década en el manejo y pronóstico del Síndrome Coronario Agudo con Elevación del ST en ancianos en Navarra

Abstract
La cardiopatía isquémica y los síndromes coronarios agudos son frecuentes en el anciano. Clasicamente en ancianos se han infrautilizado los tratamientos de eficacia demostrada, y a menudo la decisión de no tratar se base en criterios de edad. Afortunadamente estas tendencias van cambiando, lo que se traduce en beneficios pronósticos. Seleccionamos a todos los pacientes de 75 años o más, ingresados en cualquiera de los 4 hospitales públicos de Navarra, por Síndrome Coronario Agudo con Elevación del Segmento ST, durante los años 2001 y 2011, y los comparamos en términos de manejo y pronóstico. Los sujetos del periodo 2001 ingresaron menos en servicios de Cardiología y en el hospital terciario de referencia en patología cardiológica. Además , se les realizaron menos pruebas complementarias y recibieron menos tratamiento farmacológico y revascularización. Los sujetos de 2001 presentaron un peor evolución clínica, presentaron mayor mortalidad hospitalaria, mayor mortalidad en el seguimiento a un año y más reingresos. Esta peor evolución, se relacionó, al menos en parte, con el manejo médico menos intensivo.
